Courtesy Listing Procedure
Describes how UAF will help to find a new home for an animal whose owner is unable to keep him/her.

MISC FORMS
Credit Card Authorization form
Completed whenever a credit card is to be used. UAF accepts Visa, Mastercard, American Express and Discover for Donations, Adoption Fees, Spay/Neuter Deposits, Fund Raisers, etc.

Home Visit Report Completed by a UAF "Qualified Friend". FOSTERING Foster Care Record Maintained by the Foster Parent while the animal is in the foster home. ADOPTION Pre-Adoption Questions Used to help screen a potential Adopter during the interview process. Home Visit Report Completed before or after the adoption has been approved. DOG Dog Pre-Adoption Questionnaire Used to help screen a potential Adopter of a foster dog via email or fax. Dog Adoption Application Filled out when the questionnaire has been satisfactorily completed. CAT Cat Pre-Adoption Questionnaire Used to help screen a potential Adopter of a foster cat via email or fax. Cat Adoption Application Filled out when the questionnaire has been satisfactorily completed. ADOPTION EVENTS Adoption Site Supervisors Contains general guidelines for Adoption Events. General Policies and Directives Contains UAF policies and directives. Waiver Completed when non-UAF dogs are attending an Adoption Event. Relinquishment Form Filled out when an Owner or Adopter is unable to keep an animal. COURTESY LISTINGS Courtesy Listing Procedure Describes how UAF will help to find a new home for an animal whose owner is unable to keep him/her. Courtesy Listing Agreement Must be signed by the person relinquishing the Animal. Courtesy Listing Dog Questionnaire Describes the dog being relinquished and is filled out by the person relinquishing the dog.
